In reading the PDF I noted a number of typos, incorrect references, bad
formatting results, etc. The attached PDF corrects these. The XML file is
the one used to produce it. (Note this no longer contains This elements as
they are not included in is.rnc.)

When I get a spare minute I'll document the changes I have made, but don't
have time for that this evening.
