The following issue, raised by Francis Cave, did not get discussed in
Montreal (for which my apologies Francis):

The key point here is that we do not want to change the prefix, only the
URI it refers to (e.g. to go from one version of the XHTML schema to
another)
Could this be done by adding an xmlns:prefix attribute on dsrl:to
element that would define the URL of the resulting element?
If not, how does a prefix specified in the content of a dsrl:to element
get assigned a URL? Must it take the URL defined for that prefix in the
source document?
