Theoretically, there could be a use when there are Qnames in content being
used. Such prefixes would not be distinguished and available in an XML
infoset implementation, such as DOM or SAX. So URL remapping is the only
option.

I don't know whether there are any cases of this actually, and I don't
suppose it is particularly important. I agree with Murata-san's point in
general, though. So perhaps if Martin does include Namespace remapping, it
could have a note to the effect "Prefer prefix remapping to namespace
remapping."
