I have some concerns with the productions rules
pio, pic, lit, lita, name-token-group are not really defined as such
in W3C XML (but in SGML)
NCName is defined in W3C XML-Names
s should be S to conform W3C XML notation
name-token-group should be Nmtokens to conform to W3C XML notation
URI is defined in IETF RFC 3986

In A.1

The sample is visually truncated

   ...with Clause Clause 5.... (xref already provide the word Clause)
