Martin,

I would like to have the reference model (the 2-phase processing model)
in the body of this text. It should make clear how entity mappings and
entity definitions are supported.

6.3 should make clear that defaults are used only when the attribute is
absent.

Default contents (6.5) are used when "the element
is empty, or not present". But how do you detect absense of elements?

What about mapping elements to attributes and vice versa?
