As you say, it is too late for substantive changes at this stage. Only items brought up in NB ballot comments or at the ballot resolution meeting can be answered at this stage, unless Ken forces me to go to another FCD ballot, when your comments can be considered with other ballot comments.
I would, however, argue against your first point. The namespace declaration on the from element is not designed to set the current value of the namespace, but to confirm that this is the namespace that is meant to be changed when mapping to the new element. Its a safety valve to make sure that users don't get the wrong url associated with the prefix.

the text seems to suggest that the namespace declaration has to be on
the actual from element, rather than it being anywhere. If it must be on
the dsrl:from element, then there is no reason to use XML namespaces
conventions at all, you could have an attribute "namespace". I would
change the example so that the namespace declarations were on the parent
map, which I think it better practice anyway.
S6.7 Note #2. Typo? Should be "entity" not "attribute"
In the XSLT code, I think all named templates should use a namespace
prefix for their name. (I only found out that this was possible from Ken
recently.) I am thinking of adding my Schematron implementation to the
ISO schematron text in the same fashion as Martin has done for DSRL, and
that would be one of the changes I would put in.
